Delaware Hospice is actively seeking a full-time Program Representative to work in Sussex County.
We invite you explore a Delaware Hospice career as a Program Representative.
The Program Representative is geared towards building awareness of DHI services within a specific community including education to medical professionals. Conducts in-person assessment and/or admission visits to potential patients and families in a prompt, courteous manner. Focuses on identifying patient/family need for services and explains how Delaware Hospice can help them. Follows established policies and procedures to admit patients into the program. Completes visits to referral sources to obtain information/documentation that is needed for completion of the admission. Is responsible for response to customers and referral sources. This is a non-exempt position.
Qualifications: High school graduate. Seasoned customer service professional with active listening and assessment skills. Ability to work in a competitive environment. Excellent command of the English language in both verbal and written communication skills, clinical documentation experience preferred. Ability to manage interactions with a diverse population. Must have demonstrated customer service and collaboration skills; strong organization and administrative skills. Demonstrated ability to engage and disengage conversations as appropriate. Familiarity with general referral process preferred. Working knowledge of community resources. Must have ability to build relationships in a professional setting and demonstrate active listening skills. Must have clean driving record.
