Introduction
Delco Detox, an Oasis Rehabilitation Center Inc. Organization, is looking for any Allied Healthcare Professional to join our team as a Care Coordinator for our clients that struggle with chemical dependency and mental illness or co-occurring disorders at our inpatient level of care facility.
Care Coordinator Summary
Care Coordinator's role in supporting our various departments, the other professionals, and ensuring a smooth operation of health services.
Care Coordinator's responsibilities will include performing various medical, clinical, operations, and administrative tasks to contribute to the overall efficiency and effectiveness of healthcare delivery.
This position requires a strong attention to detail, excellent communication skills, and a commitment to providing high-quality patient care.
Care Coordinator's Duties and Responsibilities
Patient Care Assistance:
- Provide hands-on assistance to healthcare professionals in client care activities, such as taking vital signs or triage, performing assessments for withdrawals and other symptoms, and assisting with medical procedures.
Medical Record Maintenance:
- Maintain accurate and up-to-date patient records, ensuring confidentiality and compliance with privacy regulations.
- Input and retrieve information from electronic health record systems.
Equipment Operation:
- Operate and maintain medical equipment, such as diagnostic machines and monitoring devices.
- Report any malfunctions or issues promptly.
Administrative Support:
- Assist in managing administrative tasks, including reminding clients about their appointments, answering phone calls, and coordinating patient intake and documentation.
- Ensure a productive flow of communication with other professionals and management.
Health Education:
- Educate patients on basic health practices, medications, and follow-up care instructions.
- Provide informational materials and resources to enhance patient understanding.
Group Facilitation and Clinical Activities:
- Providing educational activities and groups that will allow the client to engage and understand the concepts about their conditions and to help them rediscover hope and confidence in their recovery.
- Monitoring clients during walks, outings, and to other offsite locations as directed by management.
Infection Control:
- Adhere to and promote strict infection control protocols to maintain a safe and sanitary healthcare environment.
- Follow established guidelines for the disposal of medical waste.
- Disinfecting medical devices and equipment.
- Promptly wash hands as directed by CDC guidelines.
- Wearing appropriate PPE as directed.
Collaboration:
- Collaborate with other healthcare professionals, including nurses, physicians, therapists, case management, administrative staff, and management to facilitate seamless patient care and communication.
Emergency Response:
- Respond promptly and effectively to emergency situations, following established protocols and providing assistance as needed.
Medication Management:
- Observing and assisting with client's medication self-administration.
- Logging in all accessible medications. Creating an inventory for each clients' medications and performing daily audits to maintain the integrity and efficiency in medication orders and refills.
Point-of-Care Testing:
- Comfortable in performing urinalysis drug screen on clients for measuring of therapeutic levels and integrity of their sobriety.
- Collecting specimens and following delivery service protocol when informed.
Observations and Rounds:
- During inpatient level of care, engaging in timely monitoring, observing, and reporting each clients' whereabouts, behaviors, vitals based on the interval of times per initial/specialize circumstances, detox, and RTC level of cares.
- Documenting appropriately and efficiently on rounds form.
Client Communication:
- Able to communicate with the client in a respectful, professional, and understandable way, in which the client feels comfortable, comprehends, and motivated to listen and follow through.
- Being able to redirect the client, relay information to the client, and diffuse client's methods of staff splitting and causing manipulation.
Professional Ethical Code of Conduct:
- Leading by example on how to maintain professional boundaries with other employees and clients.
- Refraining from profanity, inappropriate behavior, promoting alcohol, drug, and other inappropriate topics or events, and uphold self-control with negative emotions, personal affairs, and biased opinions.

Electronic Medical Record and Documentation:
- Capable of using an EMR to document a client's progress, completing assessments and screenings, able to upload documents, and input information on KIPU.
- Completing documentation for each client promptly, concisely, and detail-oriented.
Scope of Practice and Responsibilities:
- Following the full extent and limitations based on your credentials capabilities of performing certain skill sets.
- Completing work duties, prioritizing assigned tasks, and report updates consistently with management and team mates.
- Always reach out to specialist, colleagues, and/or management when engaging into a situation needing more than what you can perform.
Following Chain of Command and Professional Courtesy:
- Learning and engaging with established team mates to inform, be guided, and to address a crisis, conduct a certain task, clarification on information, needing assistance, and/or handling a situations.
- Respecting the specialists and management decision on the treatment plan for a client and enforcing it to the client and communicating to specialists and management on the clients' response, behavior, progress, and observations.
Enforcing Treatment Agreement Rules:
- Responsible to remind client's of appropriate behaviors during their stay. Redirecting clients to perform ADLs, attend groups and other activities, be medication compliant, following group schedule, respectful towards staff and peers, and to remind the clients that discipline and structure is part of their treatment and good practice for when they become independent or self-autonomous.
Self-Advocate and Transparency:
- Be mindful of your limitations and knowing when you lack training, lack understanding, and/or needing more clarificaiton when in doubt, unfamiliar, and/or handling a complicated situation, please reach out to management and express your concerns.
- As a professional it is on you, if you require more help, training, direction, input, etc., of how to handle a situation or perform a task, because if you don't make it known, then everyone will assume you understood and you can perform such tasks, when knowing that it will cause more harm than good.
- Communication and honesty is key.
Care Coordinator's Requirements and Qualifications
Education:
- High school diploma or equivalent.
Completion of a recognized allied healthcare professional program, required.
- Associate's in Science in a healthcare professional education; highly preferred.
Certification: Required to Possess at least one of these relevant certifications as such:
- Medical Assistant (MA, CMA, CCMA, RMA, NCMA, NCRMA)
- Nursing Assistant (NA, RNA, CNA, TNA, and LNA)
- Medication Technicians/Qualified Medication Aides (CMT, QMA)
- Emergency Medical Technician (EMT intern/candidate, EMT-B, EMS)
- Pharmacy Technician (CPhT)
- Patient Care Technician/Aide (CPCT/A, PCT, PCA)
Direct Patient Care:
- Home Health Aid (HHA)
- Community Healthcare Worker (CCHW)
- Caregiver (CCG)
Coaches related to Health:
- Health and Wellness Coach (CHWC)
- Nutrition Coach (CNC)

Experience:

- Excellent verbal and written communication skills to interact professionally with patients, families, and healthcare team members.

Technical/Medical Skills and Competency:
- Proficiency in using medical equipment

- Understands vital signs parameters
Comfortable identifying and performing CPR, AED, and emergency protocols.
