Introduction
Delco Detox, an Oasis Rehabilitation Center Inc., Organization is searching for one or two dedicated Care Manager for our inpatient level of care facility for detox and RTC levels of care.
Opportunity Summary
A care manager is a healthcare professional responsible for coordinating and overseeing a client's comprehensive care plan by collaborating with various healthcare providers, assessing client needs, developing personalized treatment strategies, and advocating for the client's best interests while navigating the healthcare system; essentially acting as a central point of contact for clients and their families to ensure optimal health outcomes.
Key responsibilities of a care manager may include:
- Patient assessment:Conducting thorough patient evaluations to understand medical history, current health status, social needs, and care goals.
- Care plan development:Creating customized care plans based on patient needs, including medication management, treatment schedules, and referrals to specialists.
- Care coordination:Communicating with physicians, nurses, therapists, social workers, and other healthcare providers to ensure coordinated care delivery.
- Patient education:Providing patients and families with information about their conditions, treatment options, and self-management strategies.
- Advocacy:Acting as a patient advocate to navigate complex healthcare systems, access necessary services, and resolve issues.
- Transitional care management:Coordinating care during transitions between different healthcare settings, such as hospital discharge to home.
- Monitoring and follow-up:Regularly reviewing patient progress, identifying potential issues, and making necessary adjustments to care plans.
