Responsibilities
- Provide direct support and assistance with daily living activities, including personal hygiene, meal preparation, and medication management.
- Monitor and observe patients’ conditions, documenting any changes or concerns for healthcare professionals.
- Assist clients in maintaining their independence while ensuring their safety and well-being.
- Engage clients in meaningful activities that promote social interaction and emotional well-being.
- Collaborate with healthcare teams to develop and implement individualized care plans.
- Maintain a clean and organized living environment for clients.
- Provide companionship and emotional support to enhance the overall quality of life.
Skills
- Experience in home community care or working with individuals with developmental disabilities is preferred.
- Proficiency in patient observation techniques to effectively monitor health status.
- Knowledge of CPR and first aid procedures is essential.
- Previous experience in hospice care or group home settings is advantageous.
- Strong communication skills to interact effectively with clients, families, and healthcare professionals.
