BR / hr with start date of 8 / 21
EPIC experience Preferred
EOW; 36 hours a week; 7p-730a
Crisis Credentialing approved. Must have : upon start :
- Online verification of license to include expiration date and any disciplinary actions
- Copy of Valid Driver's License
- MMR, Varicella, Hepatitis B - positive titers OR proof of labs in process
- Negative TB test within 12months of start date - PPD or QuantiFERON / TSpot / TB Gold with negative results
- completion of Standard 9 Panel Drug Screen within 12 months of start date
- National and County Criminal Background check completed within 12months
of start date - must go back 7 years to include all aliases and include the following :
o Social Security Address Check
o GSA
o OIG
o OFAC
o National Sex Offender Registry check
