Job Descriptions
Perform home health aide services by following a written plan of care.
Work is performed under the supervision of Clinical Operations.
HHA will provide personal care and related activities of daily living services for patients in their homes, skilled nursing facilities and assisted living facilities.
Reports and documentation will be prepared and regulatory and agency guidelines will be followed.
Key Responsibilities
- Reports are completed immediately in regards to abuse and neglect, according to internal and external guidelines.
- Day sheets and activity sheets are documented accurately and within established time frames.
- Care is provided in accordance with confidentiality standards, e.g. HIPPA.
- Care is provided by adhering to plan of care, which may include end of life care and the bereavement process.
- Care is provided that encourages patient's optimal level of independence.
- Care is provided using body mechanics that follow Agency guidelines.
EDUCATION / DEGREE REQUIREMENTS REQUIRED :
High school diploma or GED Licensure, Certifications or Registration Required :
Massachusetts Home Care Aide Certification required Experience
