This position is based out of Hurlock, MD, but oversees Residential Houses in Seaford, Bridgeville, Greenwood, and Georgetown areas. Day spent in Residential houses 75% and home location 25%.
Job Description
Title: Residential Coordinator
Supervisor: Delaware Director
Status: Exempt
Description: This position is primarily responsible for the case management and coordination of services received by individuals with developmental disabilities while ensuring the safety and well being of the residents in the Delaware Residential Program. In addition, this position is responsible for supervising, scheduling, training, scheduling mandated training through for Direct Support Professionals (DSPs) in the residential homes.
Qualifications:
Associates or bachelor’s degree in psychology, social work, social science, sociology, human services, or counseling; OR at least 3 years’ experience demonstrating increasing levels of responsibility and/or authority managing and supervising others.
Conditions:

- Pass criminal history background check and pre-employment and random drug/alcohol screening.
- Always maintain valid driver’s license throughout employment and maintain no more than 4 points on driving record.
Duties:
Schedule annual conference (DDDS develops “Master Schedule,” distributes draft to contractors for review and comments, to be returned within 10 working days)
- Notify all ID Team members 30 days prior to the annual meeting (to include cancellation and rescheduling of meeting if necessary)
- Make certain that all annual assessments and/or documents updates are completed and submitted by assigned discipline (e.g., “Client Fact Sheet,” “Unsupervised Time Checklist,” “Statement of Rights,” etc.)
- Attend the annual meeting.
- Serve as the individual’s advocate during the annual conference.
- Chair the annual meeting.
- Write annual meeting summary and place in the COR (no later than 2 weeks following the annual meeting)
- Ensure that all service and support tasks are assigned.
- Develop service/support plan and place in the record (no later than 30 days following the annual meeting)
- Distribute all documents as appropriate (no later than 30 days following the annual meeting)-e.g., Conference summary, plans, day service plan to DDDS, day center, family, etc.
- Send original ELP, SAU, Annual Conference Summary to HIM for archiving-statement of rights-Unsupervised Time Checklist
- Monitor/follow up on all activities and recommendations contained in the service/support/action plan.
- Directly responsible for the development and implementation of the service/support/action plan
- Make all referrals for support services, therapies, assistive tech/adaptive equipment, transportation, camp programs, special events, licensure activities, etc.
- Follow up on all activities and recommendations contained in the service/support/action plan.
- Actively monitor program and service delivery on an ongoing basis to ensure that the programs and services are provided in accordance with the services/support/action plan, and that the health, safety and well being of each resident is preserved and protected.
- Conduct monthly/quarterly case review meetings (to include recording and dissemination of meeting minutes)
- Attend monthly/quarterly review meetings.
- Make a monthly (minimum) residential site visit and document the visit in the COR.
- Make a quarterly (minimum) day program site visit and document the visit in the COR.
- Enter ID Notes (in the record) which document an ongoing awareness of the status of the resident’s life, specific services, health and safety issues, progress, etc.
- Oversee that all issues/matters concerning the health, safety, well being, services, and/or programs of or for a resident are followed to closure.
- Ensure the completion of a comprehensive monthly/quarterly review of each resident’s services and progress and write a “Monthly/Quarterly Progress Report”.
- Coordinate and facilitate the modification of program and service plans (as necessary and appropriate)
- Maintain an accurate medical appointment log in each residential site.
- Review all routine reports (e.g., “Monthly/Quarterly Progress Report,” etc.) and make recommendations/take follow-up action as necessary and appropriate.
- Maintain effective communication with ancillary service contractors to ensure that the needs of residents receiving the services are being met (e.g., PT, OT, speech, etc.)
- Respond promptly and comprehensively to all QA contacts (e.g., surveys)
- Ensure that each resident’s COR is maintained in optimum condition at all times (e.g., all information is current, entered/filed in the correct section, signed, etc.)
- Ensure that all record stripping and archiving is done in accordance with DMR guidelines.
- Ensure that all confidentiality and security of each resident’s COR is maintained.
- Maintain accurate and current PSRs in accordance with DDDS policies and procedures.
- Monitor individual bank accounts.
- Assist residents with account transactions (as necessary and appropriate)
- Ensure the prompt payment of all medical and/or other ancillary service bills.
- Copy pay stubs (monthly) and bank account statements (as received) and forward all copies to DDDS staff designated to receive the policy.
- Ensure compliance with “Individual Funds” policy
- Process paperwork for all team-approved purchases
- Monitor financial resources to maintain individual’s eligibility for financial benefits.
- Facilitate the establishment and maintenance of a positive relationship among each resident, their family, the contractor, and DDDS.
- Other duties as assigned by supervisor.
