Job Summary
The Community Health Worker (CHW) serves as a vital link between healthcare providers and the community, focusing on improving health outcomes for individuals and families. The CHW will work collaboratively with healthcare professionals to promote public health initiatives and support individuals in navigating the healthcare system.
Duties
- Travel to and attend public outreach events, with a focus on women's health.
- Provide education on health topics, including women's care and medical terminology, to empower community members.
- Assist individuals with disabilities in accessing necessary health services and resources.
- Conduct public speaking engagements to raise awareness about community health issues and available programs.
- Utilize motivational interviewing techniques to encourage healthy behaviors and lifestyle changes among clients.
- Collaborate with nurses and other healthcare professionals to ensure comprehensive care for clients.
- Maintain accurate records of client interactions and progress, adhering to confidentiality standards.
- Participate in community outreach programs to promote public health initiatives and resources.
Qualifications
- Bilingual English/Spanish or English/Haitian-Creole.
- Strong knowledge of public health principles and practices.
- Familiarity with medical terminology is advantageous.
- Excellent public speaking skills, with the ability to engage diverse audiences effectively.
- Proficient in motivational interviewing techniques to facilitate client engagement.
- Strong interpersonal skills, with a compassionate approach to client interactions.
- Ability to work independently as well as part of a collaborative team.
