Creates optional outcomes for the patient and family by managing complex psychosocial, medical, and economic co-morbidities. Through advanced practice skills mobilizes resources to reduce risk and functions as ambassador between Hospital and community. Responsible for providing high quality patient care, displaying good skills in assessment, treatment planning, individual and family intervention, and discharge planning.
