Position Summary:
Delta Community Supports, Inc. is seeking a well-organized and detail oriented program specialist with excellent communication skills. This position is located in Delaware County. Reporting to the Community Center Director, this individual provides coordination and support to participants within Delta’s Intellectual Disability/Office of Developmental Programs (ODP) service programs while also advancing Delta’s mission and values.
Key Responsibilities:
- Monitor the development and implementation of high quality individual support plans for a support list of up to 30 individuals.
- Coordinate with the Support Coordinator and planning team:
o Annually provide an assessment for the development of the ISP
o Attend and participate in all annual ISP meetings, team meetings, monthly monitoring with support coordinator, and other meetings as needed as a member of the ISP team.
o Annually update lifetime medical history and individual protocols
o Develop appropriate outcomes to support growth towards greater independence in self-care, daily living skills, socialization, and community living.
o Monitor the monthly documentation completed by Direct Support Staff of individuals’ participation and progress towards outcomes
o Ensure supervision and all support needs as per ISP and ensure AIS/SH/HCH/needs exception is applied for as needed and reviewed one month prior to the expiration of services.
o Develop quarterly reviews for each individual on support list.
- Support, motivate, empower and train the Direct Support Professionals on:
o Implementation of the ISP/ICP
o Behavior Support Plans and Social Emotional Environmental Plans
o Feeding protocols
o Supervision levels
o Conducting meaningful group activities
o Individual engagement
- Communicate with families, providers and Support Coordinators regarding the individual’s progress and participation in the program.
- Serve as primary contact person for county/state required ODP activities and communications.
- Monitor staff compliance with all State, County and Delta policies and procedures.
- Work with the Residential Coordinator to complete an Initial Assessment within thirty (30) days of an individual starting the program and annually thereafter.
- Develop and provide meaningful activities that are person-centered in accordance with the individual plans.
- Support the integration of individuals in the community.
- Support individuals with communication and involvement with family and friends.
- Maximize the development and capacity of staff to advance the mission and values of Delta.
- Ensure the full capacity of programmatic resources.
- Promote and demonstrate ethical and professional conduct at all times ensuring employees conduct
- themselves in an ethical and professional manner.
- Promote continuous quality improvement projects and ongoing performance measures.
- Other duties as required.
Qualifications
Qualifications:
- Master’s Degree or above from an accredited college or university and one (1) year work experience working directly with persons with intellectual disabilities;
Or, a Bachelor’s Degree from an accredited college or university and two (2) years’ work experience working directly with persons with intellectual disabilities;
Or, an Associate’s Degree or sixty (60) credit hours from an accredited university or college and four (4) years’ experience in the supervision of community based programs working directly with persons with intellectual disabilities and/or autism.
- Valid driver’s license in the state of residence
