Occupational Therapist (Health Programs)
Reports to: VP of Health Programs
SUMMARY:
The Occupational Therapist (OT) will work in coordination with clinic staff to help patients improve their ability to perform tasks in their daily living and working environment.
MINIMUM QUALIFICATIONS:
· Master’s Degree
· Occupational therapist licensure
· 5 years of experience.
ESSENTIAL DUTIES:
· Conduct patient assessments and evaluations to determine the patient’s physical condition, limitations, medical history and personal goals.
· Communicate with clinic staff to confirm plans of care.
· Advise patient’s family members and other supportive individuals regarding the level of care for the patient.
· Provide OT guidance, support and education during patient sessions.
· Instruct patient how to interact with adaptive technology and how to use assistive devices.
· Guide patient through graded activities.
· Generate documentation for patient and patient files
· Other duties as assigned
