The Case Manager - Teens is responsible for the facilitation of services to transition age youth (12-24 years old) who face daily obstacles such as living independently, obtaining employment, obtaining post-secondary education, and in developing supportive social and recreational activities. They are also responsible for providing case management services and mental health rehabilitation services to Severely Emotionally Disturbed (SED) clients and their families.

 

This position is responsible for observing policies, procedures and safeguards to protect the privacy of health information in compliance with the HIPAA Privacy (Privacy Rule).

 

Essential Functions:

  • Serve as a facilitator for the youth’s person-centered planning and his/her Individualized Transition Plan (ITP) team.
  • Conduct, arrange, or broker the necessary services and supports for youth according to their ITP.
  • Interface with adult mental health service coordinators to ensure continuity of age- appropriate services for youth as they approach 18 years of age and older.
  • Assist youth in acquiring access to community settings and resources (e.g., employment, housing, social security benefits).
  • Actively track and report assessments, service plans and reviews on a quarterly and annual basis to ensure the clinical team members are current with detailed case information.
  • Work collaboratively with teachers and school guidance counselors, and other service practitioners to ensure appropriate services for youth.
  • Coach youth as they progress through their levels of service need.
  • Develop crisis backup support plans.
  • Promote the involvement of parents and other informal and formal key players in planning services and supports for each youth.
  • Develop and implement strategies to overcome obstacles and challenges.
  • Manage resource allocations in accordance with agency and school district regulations.
  • Facilitate active and ongoing communication with all members of youth’s service team.
  • Complete youth medical record documentation and other required forms for each service provided, billable and non-billable, and incidents as outlined in the agency Policy and Procedure.
  • Participate in treatment planning and implementation through case management and other mental health rehabilitation services (individual and group skill development).
  • Understand and comply with all federal, state, accreditation, and agency policy, procedures, plans, program requirements and protocols regarding case management services.
  • Transport, via private or agency vehicle, youth to/from home, school, the agency, and treatment meetings, as needed.
  • Encourage family members to be active participants in the service planning and delivery process.
  • Assist children and families in accessing support within home, school and community.
  • Complete foster home visits and biological home visits as outlined in the treatment plan.
  • Ensure client confidentiality as prescribed by agency policy and federal law.
  • Other duties as required.
