Under direction of the Program Manager of Crisis Residential and within federal, state, and local guidelines provides moderately complex case management work. Work involves developing and maintaining long-term contact with clients, client families, and service providers for medical, social, educational, and related service needs. This position is grant funded.

Coordinates, monitors and provides a therapeutic environment for individuals experiencing a mental health crisis and residing within the crisis residential facility. Provides quality skills training and medication and symptom management. Assists residents in gaining access to mental health, medical, social, vocational, financial and other needed services to assist in returning the resident to their pre-crisis level. Works under moderate supervision, with limited latitude for the use of initiative and independent judgement.

Education, Training And Experience (including Licensure And Certification)

Graduation from an accredited four-year college or university with major course work in a human services field as required to qualify as a Qualified Mental Health Professional (QMHP) and one (1) year prior experience in a mental health field required. Master's Degree in a human services field and previous Crisis experience preferred. English/Spanish bilingual preferred.

Registration, Certification, Licensure Or Other Qualifications

  • Qualify as a Qualified Mental Health Professional
  • All job specific staff development training must be completed within 30 days of hire.

Knowledge, Skills And Abilities

  • Knowledge of community resources; of case management principles, objectives, standards, and methods; and of program policies and procedures.
  • Ability to assess clients' needs, to coordinate clients' services, to monitor program effectiveness, to communicate effectively.
  • Moderate mathematics skills. Proficient reading, writing, interpersonal relations and communicative skills; ability to read and comprehend simple instructions, short correspondence, and memos. Ability to write effective case management notes. Ability to effectively present information in one-on-one and small group situations. Highly effective in working objectively with a diverse group of people and must demonstrate communication, organizational, administrative and office managerial skills. Computer skills required both clinical and clerical programs. Ability to make decisions based on education preparation and or nursing/clinical experience, which reflect consistency with Agency philosophy, policies and procedures and standards of care.
  • Auditory and visual skills; ability to bend, stoop, sit, stand, reach, ability to talk and hear with aid greater than 2/3 of the time, ability to have clear vision at 20 inches or less; color vision (ability to identify and distinguish colors); peripheral vision (ability to observe an area up/down or left/right) and ability to adjust focus (ability to adjust eye to bring into focus).
  • Understanding and practice of Trauma Informed Care principles.

Essential Duties include the following. Other duties may be assigned.

  • Conducts orientation to clients upon admission, explaining facility and group rules, client rights, consents and group schedules; completes and/or coordinates the completion of documents contained in the orientation packet; ensures proper search and return of belongings; communicates and safety/medicals concerns with supervisor.
  • Completes discharge process as outlined in the discharge checklist; ensures safety plan is completed and provided to the client and emailed to the assigned Case Manager.
  • Completes and/or coordinates the treatment plan to ensure the client's needs are identified; completes and/or coordinates the psychosocial assessment within 48 hours of admission.
  • Completes and/or coordinates the discharge plan within three (3) days of admission or upon notification from the Program Manager to ensure continuity of care upon discharge, to also include the appointment and referral sheet ensuring clients are aware of scheduled appointments and to access agencies for referrals; notifies case manager of any pending needs.
  • Conducts skills training, groups related to social, financial, independent living skills, symptom management, or other relevant treatment protocols to improve quality of life for residential clients.
  • Provides thorough documentation; documents all services provided to a client or on behalf of a client in a progress note; each service delivered will be documented according to agency and stated guidelines to also include any clinically significant information and in the case of a crisis and/or emergency; documents all skills training groups for each assigned client in the client's medical record chart; documents as services occur but no later than the end of the shift.
  • Supervises self-administration of medication; practices workplace safety procedures; adheres to the use of Universal Precautions; adheres to the Center Infection Control Plan; informs appropriate staff of any safety hazard or issue promptly; completes Food Handlers training and ensure it is up to date; prepares, preps and/or monitors meals as assigned; assists with upkeep of the residence including, floors, dusting, and laundry; attends all scheduled clinical team meetings; maintains current status on all required trainings and refreshers.
