Job Summary

Under minimal supervision, the Nurse Manager of Care Management performs managerial and leadership functions over professional nursing and support care team in inpatient care management that perform the functions of utilization review and management, progression of care facilitation and transition of care coordination. Applies critical thinking, evidence-based policies, procedures, guidelines and competence to collaborate with the health care team to promote excellence in healthcare consumer safety and quality outcomes and independently works with the Director to maintain departmental budget.

Adheres to the American Nurses Association (ANA) standards of professional nursing practice / process and performance, and the ANA Code of Ethics (2015)

  • to provide care to all healthcare consumers. Applies critical thinking, evidence-based practices and competence to collaborate with the health care team to promote excellence in healthcare consumer safety and quality outcomes. Utilizes the Theory of Human Caring and Caritas in daily practice. "Jean Watson, PhD, RN, AHN-BC, FAAN, Founder / Director, Watson Caring Science Institute."

Essential Functions :

  • In collaboration with the Senior Director of Care Management, implements and directs all Hospital Care Management initiatives, policies, procedures, and processes to promote an efficient and effective Care Management program (15%)
  • In collaboration with the Senior Director of Care Management, oversees and coordinates staff activities to facilitate integrated and proactive care management processes including discharge planning, utilization review and management, progression of care facilitation, and transitions of care processes between Ambulatory and Hospital care teams.

  • Staff Management, Scheduling, Educating, Mentoring and Recruitment (15%)
  • In collaboration with the Senior Director, reviews and manages the department budget to meet or exceed budgetary expectations. (10%)

  • Ensures compliance with state, federal, regulatory and payer requirements. Policy development and revisions (10%)
  • Support and implementation of organizational initiatives and strategic planning (10%)

  • Education :

  • Bachelor's Degree Nursing Required
  • Work Experience :

  • 1-3 years Management experience within an Acute or Ambulatory Care Management Required and
  • 1-3 years Two or more years of relevant clinical experience with diverse populations in a health care setting including hospital, clinic or home health providing patient education, case management, and / or care coordination Required and
  • 1-3 years Utilization Review
  • Licenses :

  • RN-Registered Nurse - DORA - Department of Regulatory Agencies Required and
  • BLS-Basic Life Support (BLS / CPR) - AHA - American Heart Association or American Red Cross Required
  • Knowledge, Skills and Abilities :

  • Knowledge of care management concepts, principles, strategies, and processes. including tools and techniques for identification, stratification, and management of high-risk clients.
  • Adherence to Standards of Professional Nursing Practice / Process / Performance [ANA Standards addendum- signed] and accrediting body standards.
  • Possess strong program leadership and management skills. Ability to deal effectively with rapidly changing priorities with a team focus through assertive and creative problem solving, conflict resolution, and negotiation skills.
  • Ability to work effectively in a team matrix environment. Ability to communicate clearly and effectively at all levels and in a cross functional environment.
  • Ability to work closely with multidisciplinary staff, charge nurses, clinical nurse educator(s), and Nursing Managers, providers, executive leadership to provide care to patients with respect to their uniqueness using informed judgment, competence, individual qualifications, and collaboration with the health care team.
  • Possess a high level of understanding of community resources, treatment options, funding options and special programs.
  • Supports compliance with national patient safety goals, accrediting agency standards, and quality initiatives.
  • Evaluates practice in relation to professional standards, statues, regulations, hospital and departmental policies, standards of care, and regulatory
  • Promotes quality care by redefining priorities for care based on evaluation of healthcare consumer / family response or changing condition.
