Requirements
- Doctor of Medicine (M.D.) or Doctor of Osteopathic Medicine (D.O.) degree from an accredited institution
- Current, active full licensure as a physician in any U.S. state or territory
- At least one year of supervised experience providing direct service in a clinical setting, such as an internship or residency program
- Board certification in family medicine or completion of an accredited residency program within the past three years
- Three or more years of experience since completion of an accredited residency program, or actively pursuing board certification in family medicine
