The Department of State Hospitals-Atascadero is a secure public sector hospital providing inpatient forensic services for mentally ill adult males who are court committed throughout the State of California.
The Health Record Technician II (Specialist) will be directed by the Health Record Technician III in the Health Information Management Division at Department of State Hospitals - Atascadero (ASH). The incumbent will be responsible for, but not limited to, the following duties:
- Perform a variety of complex assigned duties requiring independence of actions and decision, discriminating judgment necessary to evaluate technical abstracting and coding problems, public contacts, with increased consequences of errors.
- When assigned, attend and participate in appointments for Patient Access to Medical Record.
- Photocopy requested records.
- On-Unit Health Record Technician Coordinator, requires (1) The ability to physically travel to/from and perform duties on all treatment units in the secured area of the hospital; (2) Perform on-unit documentation review, chart maintenance, thinning, and/or filing; (3) Regularly walk long distances; (4) Routinely carry/transport medical record binders (estimated at 10-15 lbs).
- Conduct accurate, structured, concurrent, quantitative audits within the secured area of the facility.
- Analyze in-house requests for patient information and formulate appropriate response.
- Discharge/Court Leave Record Processing: Analyze medical records for deficiencies when a patient is discharged or goes to court. Communicate incomplete record findings to responsible staff and take steps to have the record completed.
- Abstracting/Coding: Assign ICD-10 codes in accordance with ICD-10-CM guidelines. Abstract data from a variety of sources.
MINIMUM QUALIFICATIONS: Education: Equivalent to completion of the twelfth grade. (Additional qualifying experience may be substituted for the required education on a year-for-year basis.) AND Either I: Two years of California state service performing the duties of a Health Record Technician I OR II: Four years of increasingly responsible medical or health records experience, performing a variety of tasks including at least two years of coding, indexing, and abstracting health data. (Successful completion of an academic curriculum in medical records science in an accredited school may be substituted for the required general experience on a year-for-year basis.)
