Key Responsibilities
- Plan and coordinate hospice care in alignment with the patient's individualized plan of care.
- Collaborate with other healthcare professionals and family members to ensure comprehensive support for patients.
- Conduct ongoing evaluations of hospice services to assess their effectiveness.
- Perform thorough assessments, including history taking and physical examinations, to guide patient care.
- Consult with attending physicians regarding any changes in patients’ conditions that necessitate updates to care plans.
- Maintain accurate clinical documentation and timely submission of patient visit schedules.
- Engage in interdisciplinary group (IDG) meetings and case conferences to enhance care delivery.
- Provide education to patients and families about the dying process, care plans, and nutritional guidance.
- Supervise Hospice Aides to ensure the highest quality of care is provided.
- Participate in quality assurance and performance improvement (QAPI) initiatives.
- Exceptional assessment and problem-solving skills.
- Strong organizational abilities and effective time management.
- Proficient communication skills, both written and verbal.
- Knowledge of nursing procedures and case management processes.
- Ability to provide education and support to patients and families.
- Graduate from an accredited School of Nursing.
- Must hold a valid Registered Nurse license in Arizona.
- Minimum of one year of nursing experience; hospice experience is preferred.
- Ability to perform nursing procedures and administer medications as required.
- Valid driver's license and access to a reliable, insured vehicle.
