We are seeking a compassionate and dedicated Hospice Registered Nurse/Case Manager to join our esteemed team in Tucson, Arizona, and surrounding areas. This pivotal role involves planning, coordinating, and delivering exceptional hospice care tailored to each patient’s unique needs. As a licensed Registered Nurse in Arizona, you will have the opportunity to make a significant impact on patients and their families during crucial times in their lives.

Key Responsibilities

  • Plan and coordinate hospice care in alignment with the patient's individualized plan of care.
  • Collaborate with other healthcare professionals and family members to ensure comprehensive support for patients.
  • Conduct ongoing evaluations of hospice services to assess their effectiveness.
  • Perform thorough assessments, including history taking and physical examinations, to guide patient care.
  • Consult with attending physicians regarding any changes in patients’ conditions that necessitate updates to care plans.
  • Maintain accurate clinical documentation and timely submission of patient visit schedules.
  • Engage in interdisciplinary group (IDG) meetings and case conferences to enhance care delivery.
  • Provide education to patients and families about the dying process, care plans, and nutritional guidance.
  • Supervise Hospice Aides to ensure the highest quality of care is provided.
  • Participate in quality assurance and performance improvement (QAPI) initiatives.

  • Proficient communication skills, both written and verbal.
  • Knowledge of nursing procedures and case management processes.
  • Ability to provide education and support to patients and families.

Qualifications

  • Graduate from an accredited School of Nursing.
  • Must hold a valid Registered Nurse license in Arizona.
  • Minimum of one year of nursing experience; hospice experience is preferred.
  • Ability to perform nursing procedures and administer medications as required.
  • Valid driver's license and access to a reliable, insured vehicle.
