Your Role & Responsibilities
As a Home Health Physical Therapist, you’ll bring positive change to patients by delivering in-home therapy that restores independence and quality of life.
- Design and implement personalized treatment plans to help patients achieve mobility goals.
- Guide patients through therapeutic exercises and recovery techniques.
- Educate patients and families on maintaining long-term mobility and wellness.
- Collaborate with physicians, nurses, and the care team to ensure coordinated, high-quality care.
