Your Key Responsibilities
- Patient Assessment – Conduct comprehensive evaluations to determine patients’ home care needs.
- Care Planning – Develop and implement personalized care plans tailored to each patient’s unique situation.
- Team Coordination – Lead and guide care teams, ensuring alignment with patient goals and needs.
- Counseling – Provide education and guidance to patients and their families to support effective healthcare decisions.
- Documentation – Maintain accurate clinical records and communicate updates to primary physicians.
What We’re Looking For
- Licensure – Active and valid Arizona RN license.
- Experience – Minimum of one (1) year in home health care, with proficiency in OASIS documentation preferred.
- Transportation – Valid Arizona driver’s license and reliable transportation.
- Certifications – Current CPR certification and Arizona Fingerprint Card.
- Language Skills – Bilingual or conversational Spanish skills highly preferred.
Education Requirements
- Degree – Associate's Degree in Nursing or higher.
