Your Responsibilities
- Conduct in-home health assessments and develop personalized care plans.
- Administer medications, treatments, and wound care as prescribed.
- Educate patients and families on condition management and promoting independence.
- Monitor patient progress and adjust care plans accordingly.
- Supervise LPNs and Home Health Aides to ensure top-quality patient care.
- Participate in interdisciplinary meetings to provide comprehensive, coordinated care.
- Active RN license in Arizona.
- Minimum of one year of RN experience (home health, hospice, or critical care preferred).
- CPR certification and a valid driver’s license with reliable transportation.
- Strong communication, organizational, and interpersonal skills.
