Key Responsibilities:
- Assess and evaluate patients' therapeutic, rehabilitative, and functional statuses.
- Develop and implement comprehensive care plans tailored to individual needs.
- Evaluate home environments for safety and functionality.
- Direct and initiate physical therapy treatments and instruct family members or caregivers on aspects of the therapy process.
- Identify additional needs for home health services and make referrals as necessary.
- Prepare and submit clinical progress summaries based on goal attainment.
- Provide in-service education for nursing staff as needed.
- Supervise Physical Therapy Assistants in accordance with organizational policies and state regulations.
Qualifications:
- Bachelor’s in Physical Therapy or higher learning, from an accredited institution.
- Licensed Physical Therapist in AZ
- Minimum of one year of relevant experience; community/home health experience is preferred.
- Bilingual (English/Spanish) is strongly preferred.
- Excellent verbal and written communication skills with strong organizational abilities.
- Current CPR certification.
