Your Role & Responsibilities:
Patient Evaluation: Conduct assessments and create individualized treatment plans to restore function, improve mobility, and reduce pain. Therapeutic Interventions: Provide hands-on therapy, exercises, and treatment techniques tailored to patient needs. Patient & Family Education: Communicate treatment goals and progress while providing guidance for continued care at home. Collaboration: Work alongside a multidisciplinary team—including physicians, nurses, and other therapists—to achieve the best patient outcomes. Accurate Documentation: Maintain detailed clinical records and update the care team as needed. Ongoing Learning: Stay up to date on industry advancements and best practices.
What You’ll Need:
✔ Active Physical Therapist (PT) License – Valid in Nevada.✔ Experience Preferred – At least 1 year of home health or hospice experience is a plus.✔ Strong Communication Skills – Ability to connect with patients, families, and team members effectively.✔ Reliable Transportation – Valid Nevada driver’s license and dependable personal vehicle.✔ Team-Oriented Mindset – Ability to work independently while collaborating within a care team.
