About the Role
As an LPN at Dependable Hospice of Tucson, you’ll provide patient-focused care, ensuring comfort and support for patients and their families during difficult times. Reporting to the Clinical Coordinator and Program Director, you’ll work as part of an interdisciplinary team to deliver high-quality hospice services.
Key Responsibilities
- Assess and address patient and family needs.
- Deliver care according to physician orders and interdisciplinary care plans.
- Work collaboratively with Nurse Case Managers throughout the nursing process.
- Maintain effective communication with physicians, hospice staff, and external agencies.
- Collect and report data on patients’ physical, psychological, social, and spiritual health to RN Case Managers.
Qualifications
- Current LPN/LVN license in Arizona.
- Willingness to cover after-hours and weekend shifts as needed.
- Strong organizational and communication skills.
- Valid driver’s license, reliable vehicle, and auto insurance (as required).
- Current BLS certification.
- Preferred: Experience in hospice care.
- Ability to manage emotional stress and respect diverse lifestyles.
- Required: Arizona Class 1 Fingerprint Clearance Card (current or ability to obtain).
