About the Role
As a Licensed Practical Nurse (LPN), you will deliver compassionate, patient-centered care in alignment with physician orders and the interdisciplinary care plan. Reporting to the Clinical Coordinator and Program Director, you will play a vital role in maximizing the comfort and health of patients and their families.
Key Responsibilities
- Assess and identify patient and family needs.
- Deliver supportive care per physician orders and care plans.
- Collaborate with Nurse Case Managers to execute all phases of the nursing process.
- Communicate with attending physicians, hospice staff, and external agencies to ensure optimal care.
- Collect and report physical, psychological, social, and spiritual health data to the RN Case Manager.
Qualifications
- Current LPN/LVN license in Arizona.
- Willingness to provide some after-hours and weekend coverage.
- Strong communication and organizational skills.
- Valid driver’s license, reliable vehicle, and auto insurance (as per state/organization requirements).
- Current BLS certification.
- Experience in hospice care preferred.
- Ability to manage family emotional stress and respect diverse lifestyles.
- Arizona Class 1 Fingerprint Clearance Card (current or able to obtain).
