Position Overview
As an LPN, you’ll provide high-quality, personalized care in clients' homes. Work with a collaborative team where your expertise truly makes a difference.
Key Responsibilities
Deliver compassionate, one-on-one patient care.
Conduct assessments and follow individualized care plans.
Administer medications and monitor client health.
Communicate effectively with clients, families, and healthcare professionals.
Maintain accurate and timely documentation.
Collaborate with the healthcare team to support positive outcomes.
Qualifications
Active LPN License (Arizona or Compact).
Valid Driver’s License with reliable transportation and auto insurance.
BLS Certification (in-person course required).
Current TB Test and health clearance.
Fingerprint Clearance Card (required by the State of Arizona).
Article 9 Training (we’ll schedule this for you).
