Southeastern Virginia Training Center is hiring a Social Worker to join our team. The Social Worker supports the facility’s mission by facilitating the admission and discharge process for individuals, serving as agency liaison to families, community agencies, and state or private institutions, while advocating on behalf of the individual regarding benefits and rights, and counseling individuals or family members as needed.

About the Role

Case Management

  • Coordinate Admissions and Discharges in collaboration the Community Integration and Discharge Coordinator.
  • Responsible for pre-admission summaries and other appropriate admission documentation, social histories, pre-discharge and discharge plans, placement/ discharge/transfer summaries/memos and other reports utilizing existing records and personal contacts.
  • Draft and prepare Pre-Move and Annual Discharge Plan and Discussion Record (DPDR) within timeline.
  • Plan and facilitate process for individual’s transition/ admission/placement/discharge, in collaboration with Personal Support Team members, AR/Guardian, CSB.
  • Complete Quarterly Social Work Summaries documenting family’s preference concerning community integration.
  • Complete VIDES assessments according to schedule.
  • Complete FRC as scheduled.
  • EHR: Data and information will be accurately, efficiently and timely recorded for each individual using the Electronic Health Record.
  • Prepare the Annual/Comprehensive Social Worker Assessment Reports; Attend and participate in annual assessment meetings.
  • Ensure documents have been mailed to families in a timely manner. Ensure we receive those documents back to remain in compliance.

Participate I Specialize Team Meetings

  • Participate in Personal Support Team Meetings.
  • Participant on Community Integration Planning Team.
  • Facilitate pre-move meetings for individuals in the active discharge process.

Correspond with Public/Private Agencies and Families

  • Contact public/private agencies to include the Community Service Boards, concerning discharge planning efforts via telephone, email, or mail correspondence.
  • Communicate with individual’s medical insurance providers to verify coverage and assist to rectify billing issues when appropriate.

Counsel Individuals and/or Families

  • Present individual’s rights every 365 days and answer any questions regarding status.
  • Communicate and educate families (Authorized Representatives/Legal Guardians) on appropriate community integration options regarding options related to admission and discharge.
  • Accompany families and individuals with tours of potential placement options and transitional visits.
  • Assist with ensuring residents maintain frequent communication via video calls with families when SEVTC’s visitation restriction is in place.
  • Follow-up with AR/Guardian authorizing any changes regarding visitation or release of information.
  • If a DNR order is in place, follow-up with AR/Guardian to confirm decision and communication with team accordingly.
  • Deliver mail/packages to individuals.

Secure Financial Benefits

  • Assist Reimbursement/Medicaid Tech in securing benefits
  • Follow-up with Social Security office regarding overpayment and eligibility
  • Review residents’ PFA balance semi-annually or as needed to make request for expenditures with Residential Managers.

This position is considered essential personnel and would be required to work in the event of an emergency or facility need.

We seek a candidate with following:

Minimum Qualifications

  • Mid-level experience as a paid Social Worker, extensive experience in case. management and working with individuals who have an Intellectual Disability.
  • Intermediate skill level in the use of Microsoft Office (Word, Excel, PowerPoint, Access).
  • Demonstrated skill and proficiency with data entry and database management.
  • Valid active driver’s license.
