Responsibilities
- Conduct thorough patient assessments to determine care needs and develop individualized care plans.
- Administer medications and monitor patients for adverse reactions or side effects.
- Perform various clinical procedures, including venipuncture, suture placement removal, tube feeding, wound care treatments and pain management.
- Collaborate with interdisciplinary teams to provide comprehensive care in a teaching skilled nursing home setting.
- Educate patients and families on health management, treatment plans, and preventive care practices.
- Maintain accurate patient records using point click care software to ensure continuity of care.
- Advocate for patient needs and coordinate with healthcare providers to enhance patient outcomes.
Requirements
- Current Registered Nurse (RN) license in the state of practice.
- Proven experience in a clinical setting; experience in patient & wound care is preferred.
- Proficiency in performing venipuncture, medication administration, pain management techniques, and other nursing procedures.
- Familiarity with point click care or similar electronic health record systems is advantageous.
- Strong critical thinking skills and the ability to work effectively under pressure.
- Excellent communication skills for effective interaction with patients, families, and healthcare teams.
