Deschutes County Health Services supports over 400 staff in Behavioral and Public Health programs working to promote and protect the health and safety of the community in a broad variety of community-based settings.

This position is part of Public Health service area, which works to keep people healthy and safe by striving to improve and protect the health of people in Deschutes County. This work includes: disease prevention, surveillance and control; food and consumer safety; assuring clean and safe drinking water; ensuring access to essential health services; providing education about healthy lifestyles; implementing strategies to reduce and prevent high-risk behaviors among youth; and responding to pandemics and emerging threats, outbreaks and other events.  

Prevention and Health Promotion (PHP) is a section of the Public Health Service Area. Programs within PHP work with community partners and community members to create conditions that equitably improve health and well-being for all. PHP staff create a dynamic team of people working toward promoting and protecting the health and safety of our entire community.

ABOUT THE JOB
We are currently seeking a candidate to fulfill the role of Community Health Specialist II.  This position will act as the Regional Health Systems Change Coordinator for the Tobacco Prevention and Education Programs (TPEP) in Crook County, Deschutes County, and Jefferson County.  The position is responsible for the coordination of region-wide initiatives to prevent and reduce the use of tobacco amongst persons utilizing the health care system. This will involve working closely with Crook County Public Health, Deschutes County Health Services, and Jefferson County Public Health and external partners that provide tobacco cessation services for patients and employees.  This is an exciting opportunity to support policies, systems and environmental changes in order to improve the health and well-being of people. The work occurs in an office setting with a high level of collaboration with regional TPEP colleagues, health care system providers and community partners. This is a 40 hour per week commitment which is flexible to set within the timeframe of Monday through Friday, 8-5, with Supervisor approval. This position must travel between Crook County, Deschutes County, and Jefferson County and have office hours in all three department locations.

Key Responsibilities: 
  • Planning, directing, and supporting local TPEP coordinators to enact health systems change initiatives related to reducing tobacco-related health disparities, and increasing tobacco cessation in the tri-county region, including but not limited to the promotion of the Oregon Tobacco Quitline, increasing closed loop e-referrals, dissemination of referral data, and development of training and materials.
  • Engaging healthcare providers and systems in tobacco cessation efforts including, but not limited to, perinatal care, behavioral health and other tobacco cessation.
  • Developing and maintaining effective relationships/partnerships with internal and external partners and working in collaboration with these partners and others toward the achievement of the grant goals and strategies.
  • Contributing subject matter expertise at state and regional meetings.  
  • Planning, organizing, preparing, and distributing information, educational materials, and training sessions based on best practices in tobacco control and prevention.
What You Will Bring:
Knowledge of or experience with:
  • Familiarity with tri-county region (Crook, Deschutes, and Jefferson counties).
  • Basic understanding of electronic health records data (e.g., Epic).
  • Local community resources and regional community issues.
  • Community Health Education and behavior change theories, principles, and methods.
  • Community outreach and determining health needs.
  • Design initiatives spanning community systems (such as schools, law enforcement, health care, etc.).
  • Customer service and public relations methods and practices.
Skill in:
  • Interpreting and applying County, state and federal statutes, codes, rules, and regulations.
  • Working effectively with others to develop solutions for community health problems.
  • Training and educating employees and the general public in small or large group settings.
  • Developing and managing reports, grants, budgets, and correspondence.
  • Project management and evaluation.
  • Strategic planning and assessment.
  • Assessing and prioritizing multiple tasks, projects, and demands.
  • Establishing and maintaining effective working relationships with co-workers, other County employees, and representatives from other regional and federal agencies.
  • Effective verbal and written communication.
  • Coordinating intern(s), delegating tasks, and coaching to improve performance.

$6,144.60 to $8,234.36 per month for a 172.67-hour work month. Excellent County benefit package when eligible. This union-represented position is available immediately.

  • Bachelor’s Degree in a health or education field; 
  • AND three (3) years of related experience; 
  • OR any equivalent combination of training, education, and experience that provides the required skills and knowledge to perform the job.  
Preferred Qualifications:
  • Knowledge of tobacco cessation resources, and federal, state and county laws, codes, rules, and regulations governing public health.

Driving is a requirement for this position. Possession of or ability to obtain a valid Oregon driver's license within 30 days of hire date. The employment offer will be contingent upon presentation of an acceptable and verifiable driver's license, pre-employment screening for criminal history, driving history, and controlled substances (NOTE: Positive test results for marijuana use may result in rescission of a contingent offer of employment). This screening must be completed with satisfactory findings in order for a formal offer of employment to be extended.

Must have the ability to work a flexible schedule when necessary, to meet client and department needs.

May require certification(s) or successful completion of additional training to perform the job requirements and duties at this level within assigned program(s)
