Location: Desert Haven Enterprises
Schedule: Full-Time (37.5 hours/week) | Monday Friday, 8:00 AM 4:30 PM

Desert Haven Enterprises is seeking a dedicated and compassionate Case Manager to join our team and support individuals with developmental disabilities through person-centered services and case management. Key Responsibilities

Provide person-centered case management services for individuals enrolled in our Work Readiness and Supported Employment Programs.

Facilitate consumer admission, orientation, evaluations, and the development of Individual Habilitation/Service Plans (IHSPs).

Act as the Interdisciplinary Team chairperson, coordinating annual reviews, meetings, and discharge planning.

Serve as liaison between the agency and parents, care providers, Regional Center counselors, and other stakeholders.

Document and respond to consumer incidents and emergencies, including completing reports and follow-ups.

Maintain consumer case records in compliance with CARF standards and agency policies.

Participate in community outreach, in-service training, and interdepartmental coordination.
Qualifications

High school diploma or equivalent required; additional education in human services or related field preferred.

Experience working with individuals with developmental disabilities is a plus.

Strong communication, organizational, and problem-solving skills.

Ability to operate office equipment and perform computer-based documentation.

Must be able to respond to emergency situations appropriately.

Valid drivers license and insurable driving record (if driving is authorized).
