What are the responsibilities and job description for the LVN, Field position at Desert Oasis Healthcare?
The LVN is responsible for providing direct patient care under the supervision of a registered nurse. Responsibilities include following the plan of care, providing treatments, reporting changes in a patient’s condition to the Case Manager and practitioner, and working collaboratively with the members of the team in order to meet optimal patient care outcomes.
- Provides nursing care under the supervision of a Registered Nurse and according to the plan of care derived from physician’s orders and Registered Nursing Assessment. Nursing care includes physical care, emotional support and patient/family teaching.
- Recognizes, reports and documents symptoms that indicate changes in patient condition to the Case Manager or Clinical Manager.
- Follows the plan of care as initiated by the Registered Nurse and/or practitioner which establishes patient goals and incorporates therapeutic, preventative and rehabilitative nursing actions and initiates as appropriate.
- Assures accurate data collection is performed and documented each visit for every patient while following the patient's plan of care.
- Provides direct care to patient with problematic catheters, wound care, etc. Reports any need, change or concerns to the Case Manager, Clinical Manager or Certified Wound Care Specialist as appropriate.
- Provides health care teaching to patient and/or caregiver (this includes self-care techniques) as appropriate per plan of care. Education will utilize terms and teaching methods that are customized to the learning needs of the patient and/or caregiver.
- Provides accurate and timely documentation consistent with the plan of care; This may include preparing clinical and progress notes as appropriate. Documentation will be completed prior to the end of the scheduled shift.
- Maintains a continuing knowledge of competencies related to the nursing profession by participation in inservice and educational programs.
- When necessary participates in performance improvement projects within scope of knowledge, as requested by Clinical Manager or Director of Patient Care Services.
- Performs other duties as assigned.
- Current California LVN License.
- One year of prior professional nursing experience.
- Basic computer skills.
- Ability to manage time and stay organized. Excellent observation, verbal, and written communication skills, problem-solving skills, nursing skills.
- Good communication skills, detail oriented, critical thinking and problem solving skills. Excellent customer service and telephone etiquette skills.
- BLS required within 60 days for hire.
- Must have own vehicle as position requires travel.
- Current valid California Driver's License and proof of automobile insurance.
