The Admission RN is a Registered Nurse whose primary responsibilities include determining eligibility and initiating a comprehensive plan of care based on the needs of the patient.

- Evaluates patients for eligibility for Home Health services utilizing medical records and Medicare guidelines. Collaborates with Administrator, Director and/or Medical Director as appropriate.
- Provides patients with an explanation of Home Health services, reviews patient's rights and responsibilities and obtains informed consent.
- Develops a Home Health plan of care incorporating patient goals and expected outcomes of treatment.
- Documents a thorough physical, psychological and social assessment, incorporating assessment data into the Home Health plan of care.
- Initiates patient education and includes patient, family and caregiver in the education process.
- Develops and initiates a patient-centered plan of care based on patient's needs and goals of treatment. Arranges for consultation by other disciplines as needed.
- Performs a thorough medication reconciliation during the admission process, identifying duplications in therapy and providing patient with medication instruction.
- Ensures a smooth transition of new Home Health patients to assigned Case Manager.
- Performs routine Home Health visits as needed under the direction of the Case Manager/Clinical Manager. Any changes in patient needs or status is communicated to the Case Manager/Clinical Manager and Physician as appropriate.
- Perform other duties as assigned.

- Current California RN license.
- Home Health or Hospice nursing experience preferred.
- Knowledge of state and federal regulations relating to Medicare participation preferred.
- Excellent observation, diagnostic, medication management and clinical documentation skills.
- Able to prioritize and multi-task effectively/efficiently and work as a team.
- Works well independently.
- BLS required within 60 days of hire.
- Driving required. Current California Driver's License and proof of insurance.
