Licensed Practical Nurse (LPN/LVN) – Full-Time

About the Role:
As part of our team, you'll provide exceptional care to patients, following physician orders, adhering to industry best practices, and upholding our supportive company policies. You'll play a key role in creating a safe, positive, and comfortable environment for patients during their care journey.
What You'll Do:
- Be a patient champion! Ensure their rights are respected and they receive the best care possible.
- Stay organized by completing documentation on the same day and submitting it into the EMR within 24 hours.
- Conduct EOBs, gather required consents, and submit to intake ASAP—teamwork makes the dream work!
- Supervise and guide nursing assistants (where state regulations allow).
- Administer medications and treatments with care, ensuring everything is on point.
- Monitor patient responses to care and medications—your attention to detail ensures the best outcomes.
- Collaborate closely with the RN case manager or Clinical Service Manager to ensure top-notch care.
- Stay current with ongoing training on healthcare trends and best practices—because learning is key!

What We Need From You:
- A state LPN/LVN license (required).
- 1-2 years of hands-on nursing experience or an equivalent mix of education and experience.
