Location: High Desert/Inland Empire (In-Office Position with Occasional Travel)

Schedule: Monday - Friday, 8:30 AM - 5:30 PM

Position Overview
The Hospice Staff Scheduler (LVN) is responsible for coordinating hospice care schedules, ensuring Medicare compliance, and supporting our hospice care team. While this role is primarily office-based, there may be occasional travel in the High Desert area to conduct PRN (as-needed) or death visits when a per diem nurse is unavailable. This may occur once a month or not at all, but we need someone who is comfortable with this responsibility and has reliable transportation.
Key Responsibilities
- Schedule and coordinate hospice care teams, including RNs, LVNs, CHHAs, MSWs, and Chaplains, ensuring compliance with Medicare requirements.
- Maintain weekly schedules, manage planned vacation coverage, and handle last-minute call-offs.
- Step in as the Intake Coordinator when needed and provide field support if required when nurses are unavailable.
- Conduct PRN or death visits in the High Desert area if a per diem nurse is not available.
- Communicate daily with clinicians to confirm their schedules and manage urgent patient needs.
- Provide high-quality patient care coordination in the High Desert and Inland Empire areas.

What We're Looking For
- Licensed Vocational Nurse (LVN): Active LVN license required with a minimum of one year of experience.
- Scheduling or Hospice Experience: Preferred but not required; we value transferable skills and a willingness to learn.
- Reliable Transportation: Must have reliable transportation for occasional travel within the High Desert area to conduct PRN or death visits.
- Strong Organizational Skills: Ability to manage multiple tasks in a fast-paced environment.
- Team Player: Willingness to take on-call responsibilities during one holiday, as part of our shared team approach to support our patients and each other.
- Comfortable in the Field: Must be comfortable conducting PRN or death visits as needed, with compassion and professionalism.
