Job Summary:
The Patient Support Liaison is responsible for fostering strong relationships with referral sources, educating the community about hospice services, and ensuring seamless coordination for patients entering our care. This position requires a compassionate communicator who understands the sensitive nature of hospice care and can provide clarity and support to patients and families during critical times.
Key Responsibilities:
- Build and maintain relationships with healthcare professionals, hospitals, and long-term care facilities to promote hospice services.
- Serve as the primary point of contact for patients, families, and referral sources, providing education and support regarding hospice care options.
- Facilitate patient admissions by assisting with paperwork, coordinating with medical teams, and ensuring a smooth transition into care.
- Conduct outreach efforts in the community by attending networking events, healthcare meetings, and educational sessions to increase awareness of hospice benefits.
- Collaborate with the interdisciplinary team to ensure continuity of care and effective communication between providers, families, and staff.
- Monitor and track patient inquiries and referrals to evaluate trends and identify opportunities for community education and engagement.
Qualifications:
- A minimum of one (1) year of experience in a hospice setting is required.
- In-depth knowledge of hospice care, including services, regulations, and the admission process.
- Excellent communication and relationship-building skills, with the ability to engage healthcare professionals and families with empathy and professionalism.
- Highly organized and detail-oriented with the ability to multitask in a fast-paced environment.
- Proficiency in Microsoft Office and experience with electronic medical records (EMR) systems preferred.
- Ability to travel locally as needed for patient visits and community outreach.
Performance Expectations:
- Referral Goal: Achieve a minimum of 5 patient referrals per month to support the growth and outreach objectives of Desert View Hospice. This is a consistent expectation, with no requirement to exceed this number.
