Work Schedule:
- 3 x 12-hour shifts per week (4 x 10-hour shifts are also available)
- 3 weekend shifts per month
- Weekend shifts are included as part of the scheduled shifts (not extra)

- We are looking for a compassionate Primary and Urgent Care Provider (MD, DO, APRN, NP, PA) to join our team of highly skilled clinical providers in locations across Connecticut.

Responsibilities:
- Providing and managing immediate patient care, including physical examinations, evaluations, assessments, diagnoses, and annual well-visits.
- Engaging with patients and their families or caregivers to understand and monitor self-management care plans.
- Examining patients, evaluating medical histories, addressing patients' questions and concerns.
- Ordering, performing, and interpreting diagnostic tests and explaining the results to patients.
- Documenting individualized care plans and care coordination in the patient chart.
- Analyzing patients’ medical history, medication allergies, physical condition, and examination to determine the best care.
- Updating patients' charts and information to reflect current findings and treatments.
- Prescribing pharmaceuticals, other medications, and treatment regimens as appropriate to assessed medical conditions.
- Refer patients to specialists or to internal medicine as appropriate.
- Directing and coordinating patient care activities of nursing and support staff.
- Participating in meetings to evaluate practice processes and workflows pertaining to patient care and quality improvement efforts.
- Participating in health promotion, education, and/or prevention programs, such as PCMH and ACO.
- Following established policies, procedures, and objectives, continuous quality improvement objectives, and safety, environmental, and/or infection control standards.
Requirements:
- Active medical licensure and ability to prescribe medications in CT
- Knowledge and experience in diagnosing and treating a wide variety of illnesses.
- Primary care, Urgent care, Internal Medicine, Family Medicine experience is a plus
Supplemental pay:
- Bonus structure available based on productivity

Pay: $130,000.00 - $250,000.00 per year
