Work Schedule:
- Flexible schedule based on patient needs and facility requirements.
- Some weekend coverage may be required.
Role:
- Perform comprehensive wound assessments, develop individualized treatment plans, and monitor healing progress.
- Provide advanced wound care treatments, including debridement, infection control, and dressing applications.
- Collaborate with facility nursing staff, caregivers, and primary care teams to optimize patient outcomes.
- Educate patients, families, and caregivers on wound prevention and care management.
- Maintain accurate and timely documentation of patient encounters.
- Travel between assigned SNFs, ALFs, and home visit locations as required.
- Stay current with best practices, treatment protocols, and advancements in wound care.
Requirements:
- Active NP or MD license in Connecticut with prescriptive authority.
- Experience in wound care preferred, but training will be provided for motivated candidates.
- Ability to work independently in skilled nursing, assisted living, and home care settings.
- Strong communication and collaboration skills.
- Willingness to travel between multiple care locations.
Supplemental pay:
- Bonus structure available based on productivity

Pay: $125,000.00 - $220,000.00 per year
