What are the responsibilities and job description for the Foster Home Staff position at Destination Independence LLC?

- Maintain records of patient care, condition, progress, or problems and report/discuss observations with the Case manager.
- Interact, support, and converse with clients to keep them mentally healthy and alert.
- Provide clients and families with emotional support and instruction in areas regarding living independently, or adapting to disability.
- Perform various duties and needs as requested by clients, such as obtaining household supplies or running errands.
- Accompany clients to doctors' offices or on trips outside the home, and provide transportation, assistance, and companionship.
- Services will be provided to all clients when needed.
- Prepare healthy meals, according to diet and or medical needs ( diabetic, weight loss, etc).
- Assure proper attire according to the medical doctor's recommendations, and climate.
- Report any maladaptive Behaviors
- Report any suspected Abuse Neglect and Exploitation to the Case Manager immediately.
- We provide services that assist an individual in gaining access to needed medical, educational, and/or other services, including Transportation to Day Habilitation / Work location / Teaching ADLs according to a developed plan to reach a higher level of Independence.
- Approach individuals with a positive attitude to build a higher level of self–esteem.
- Recreational / Community activities, with prior approval
