The Social Worker / Case Manager supports individuals and families in accessing resources and services to address their social emotional and economic challenges. This role involves assessing client needs creating care plans and advocating for client wellbeing.
Key Responsibilities :
- Conduct assessments to identify client needs challenges and goals.

Requirements

Bachelor s degree in social work, psychology, or a related field (Master s preferred). Minimum of 2 years of experience in case management or social work.
