What are the responsibilities and job description for the Intake RN- Saturday-Sunday 7PM-7:30AM position at Destiny Springs Healthcare LLC?
Summary:
Under the clinical supervision of CNO/ADON and management of Director of Intake, the Intake RN provides patient assessments to ensure patients are medically clear for admission, conducts medical screenings, completes nurse-to -nurse reports for each patient transfer, and maintains accurate documentation per policy and licensing standards.
ESSENTIAL FUNCTIONS:
- Conducts medical screening exams (MSE) on all persons seeking a mental health or detox assessment. Assists team members in proactively managing requests for admission and facilitates throughput of patients. Abides by EMTALA according to regulatory requirements. Prioritizes referral packets and completes nurse-to-nurse reviews to ensure patients are medically cleared for medical transfer. Assesses patient’s immediate and ongoing physical, psychological, developmental, and cultural needs. Reviews patient history with patient/family and assures completion within appropriate timeframe. Reviews diagnostics and laboratory data and reports abnormal results to the physician(s) and other appropriate caregivers. Completes assessment andreassessments according to patient need and as outlined in policy. Administers medications and accurately reports in patient’s medical charts/records. Conducts Modified Overt Aggression Scale (MOAS) scoring for all high acuity patients. Measures and records patients’ vital signs such as height,weight, temperature, blood pressure, pulse, blood alcohol level (BLA), skin checks, and respiration. Collects, inspects, and initiates a medication verification with a patient’s pharmacy Recognizes patients that are exhibiting aggressive behavior, acting as a danger to themselves or others, or are in crisis constituting a medical emergency. Provides intervention including redirection of patients, ordering and participating in seclusion and physical restraint, and/or obtaining an order from a provider to administer chemical restraint. Conducts EOC rounds and reports all safety risks, work orders, and inventory/supplies. Obtains admission orders, including medication and precautions orders, from the assigned psychiatric and/or medical provider and documents in patient’s medical chart. Utilizes “Mindability-Roadmap to Peace of Mind” curriculum to engage and enhance the assessment process. Perform other functions as assigned by Director of Intake and Assessment, CNO, ADON, or administration.
Requirements:
- Arizona State issued Registered Nurse required.
- RN with multi-state issued license and/or Nurse Licensure Compact (NCL) must apply for an Arizona issued license within sixty days of employment. * * Two years of behavioral health experience preferred * * Level 1 Fingerprint Clearance Card required * * BLS/CPR and CPI Certification required * * Two years of Intake experience preferred
