Essential Duties and Responsibilities
- By providing valuable information, guidance and assistance using a personalized approach that will empower the client to make informed decisions that meet their needs and preferences so that the client thrives in their community.
- Conduct comprehensive assessments to gather necessary information, to ensure services, support options, and referrals align with client's needs, preferences, values and circumstances.
- Assisting with application for services (Medicaid, applying for Medicare, SNAP assistance, SER, Nursing facility level of care, financial and medical assistance, Social Security and SSI benefits, ensuring all information is accurate and submitted correctly.
- Provide follow-up to ensure services meet the client's goals and preferences
- Engage in community outreach and marketing initiatives to strengthen and promote awareness in the community
- Rapport-building to develop and maintain resources that will enhance the delivery of options counseling services.
- Must research and find solutions to complex problems such as navigating eligibility questions and finding the appropriate services for clients.
- Act as an advocate on behalf of clients and their support network.
- Must work with diverse populations, demonstrating respect for different cultural backgrounds, values and communication styles.
