Description
Supervise and coordinate with Care Managers for the assessment and delivery of home and community-based services. Act as liaison between DAAA and Intregrated Care Organizations. Develop and implement policies and procedures to ensure effective service delivery. Oversee caseload assignments, monitor field activities, assist in grievance resolution, establish goals for staff, and assist in management of Quality Improvement activities.
Requirements
Master's level licensed Social Worker / Licensed RN in the state of Michigan with BSN
Experience :
- Minimum of 5 years management experience.
- Knowledge of Medicaid and Medicare regulations is an asset.
- Ability to train, guide / coach registered nurses and social workers.
- Previous experience in Quality Improvement Initiatives is preferred.
- Bilingual preferred.
- Prior management experience preferred
Knowledge of nursing / or Social Work principles and practice.
