Under the general supervision of the Direct Service Outpatient Administrator or the Site Director, the Intake Clinician is responsible for providing initial assessments and evaluations for clients who present for same day services. This includes completing crisis assessments and making clinical determinations of appropriate levels of care. 

 

PRINCIPAL DUTIES AND RESPONSIBILITIES:

  • Provides initial assessments and evaluations to clients who present for services. 
  • Conducts intake assessments by gathering information, completing various assessment tools, writing practical comprehensive assessments and formulating diagnoses with complimentary introductory treatment plans to determine the appropriate level of care.
  • Gathers required clinical and demographic data from the patient.
  • Conducts clinical assessments, develops diagnoses and participates in developing person/family centered treatment plans with clients and their family.
  • Maintains up to date client records including written evaluations, treatment plans, quarterly reviews, termination summaries and progress notes. 
  • Authorizes medically necessary services for all clients and assigns appropriate levels of care.
  • Coordinates with treatment team staff for admissions and provides supporting documentation. 
  • Provides clinical assessments, case assignments and comprehensive outpatient counseling to individuals in need of mental health services.
  • Makes referrals (based on assessments) to the most appropriate programs and agencies to help clients access the appropriate level of care to best meet their treatment needs.
  • Works closely with clients in the decision-making process and helps them understand the benefits of accessing the appropriate levels of care.
  • Provides crisis intervention as needed by assessing risk, developing safety plans, reporting abuse or neglect and coordinating hospitalization or other emergencies. 
  • Follows the Open Access model.
  • Provides brief therapeutic interventions. 
  • Works with team members, community partners and service organizations to collaborate care. 
  • Provides crisis interventions, assessments and resource planning to adults, children and their families and determines the appropriate level of care.
  • Completes timely and accurate documentation of services in the medical record. 
  • Performs related duties as assigned.

  • Knowledge of DWIHN policies, procedures and practices.
  • Knowledge of the DWIHN provider network and community resources.
  • Knowledge of the Michigan Mental Health Code.
  • Knowledge of MDHHS policies, rules, regulations and procedures.
  • Knowledge of Federal policies, rules, regulations and procedures as it relates to DWIHN.
  • Knowledge of MHWIN.
  • Knowledge of the LOCUS model.
  • Knowledge of the clinical care process (screening, assessment, treatment planning, case management, and continuing care).
  • Knowledge of and ability to use screening and assessment tools for behavioral health services.
  • Knowledge of the continuum of care for all disability designations (SED, SMI, Co-Occurring Disorder).
  • Knowledge of Outpatient and Co-Occurring services.
  • Knowledge of the Open Access Model.
  • Knowledge of Integrated Care services for behavioral health patients.
  • Knowledge of Medical Necessity Criteria for Behavioral Health Services.
  • Knowledge of the Federal Confidentiality Regulations, 42 CFR, Part 2. 
  • Knowledge of Motivational Interviewing.
  • Knowledge of documents / regulations that govern the provision of mental health services, e.g., Medicaid Manual Mental Health and Substance Abuse Chapter III, State Plan for Medicaid, Michigan Department of Health and Human Services Quality Plan, BBA requirements and the Mental Health Code.
  • Knowledge of the practices and principles of psychological, emotional, and sociological assessment and diagnosis.
  • Knowledge of crisis training and experience in de-escalation, i.e., suicide prevention, development of crisis plans, trauma informed care, etc.
  • Knowledge of diagnostics, psychopharmacology and supportive treatment approaches.
  • Knowledge of the identification and treatment of co-occurring mental health and substance use disorders. 
  • Knowledge of community mental health services and resources.
  • Knowledge of the DSM-IV Statistical Manual.

REQUIRED EDUCATION:

A Master’s Degree from a recognized college or university in the Human Services, the Social Services, Psychology, Counseling, Social Work or a related field.

REQUIRED EXPERIENCE:

Four (4) years of professional clinical experience in a behavioral healthcare or mental health setting.

REQUIRED LICENSE(S).

A Valid State of Michigan clinical licensure:  LMSW, LMHC, LPC, LLP or PhD. 

(A Limited License will be accepted).

A valid State of Michigan Driver’s License with a safe and acceptable driving record.

 

WORKING CONDITIONS:

Employees in this classification will work on-site at a Direct Service Outpatient Clinic or the Crisis Care Center.  This is not a hybrid or remote position.
