Developmental Disabilities Resource Center is seeking to hire a full-time Early Intervention Case Manager / Service Coordinator who resides in or around Summit County to provide service coordination to children (birth-3) who are enrolled in the early intervention program in Summit County.
Compensation
- Salary Range : $28-$32.20 DOQ (for those residing in Summit County)
- 500 Hiring Bonus
- 36 hours per week / 4 days per week - typically Mon-Thurs

A DAY IN THE LIFE AS AN EARLY INTERVENTION CASE MANAGER / SERVICE COORDINATOR
As a Early Intervention Case Manager / Service Coordinator, you are responsible for the planning, coordinating, and monitoring of Early Intervention (EI) services provided to children with developmental delays and their families. This position facilitates the Family Assessment and the Individualized Family Service Plan (IFSP) process with optimal participation of the family and extended IFSP team. The Service Coordinator serves as the primary contact for families from referral to the child's exit from early intervention services at age three (3). This position also informs families of their rights and procedural safeguards and how to exercise them.
