Developmental Pathways (DP) has an opportunity for a Case Manager - Coverage in our Systems Navigation department who will manage caseloads in coverage to ensure support is provided, aligning with rules and regulations. This role will also complete coverage tasks for individuals in coverage including required documentation related to screening and referral, eligibility determination and assessments, support plan development, PMPM activities, monitorings, and other tasks as needed. In addition, this role will provide support with coverage management, research, and assignments and will work with Program Manager to determine needs amongst coverage. This role will act as a systems navigator, assessing an individual's overall needs and assisting with exploring programs, long-term services and supports, and other community resources to meet needs.
This is a 40-hour per week, Tuesday through Friday, hybrid position, with the benefit of working remotely and in-person coming into our headquarters office in Aurora, CO and/or off-site locations.

Requirements
-
Bachelor’s Degree; or
-
Five (5) years of experience in the field of Long-Term Services and Supports; or
-
(LTSS), which includes Developmental Disabilities; or
-
Some combination of education and relevant experience appropriate to the requirements of the position

Valid Colorado motor vehicle license and provide proof of motor vehicle insurance
-
Meet our driving requirements - no suspensions and no more than two (2) tickets in the past three (3) years
Preferred

One (1) year of experience in developmental disabilities/long-term services and supports
Additional Responsibilities

Manages caseloads in coverage to ensure support is provided, aligning with rules and regulations
