We are actively seeking a highly skilled and experienced Nurse Practitioner (NP) or Physician Assistant (PA) with a strong background in wound care. Key Responsibilities:
- Deliver advanced wound care treatment in patients' homes, skilled nursing facilities, assisted living centers and/or our wound care clinic.
- Manage high-volume patient caseload with precision, efficiency and compassion.
- Work collaboratively with vascular specialties, podiatric surgeons, and other providers to address the root causes of chronic wounds.
- Build and maintain strong relationships with skilled nursing facilities, home healthcare agencies, and referring providers to enhance patient access to top-tier care.
- Utilize cutting-edge wound care techniques, vascular diagnostics, and evidence-based protocols to heal wounds faster and prevent recurrence.
Requirements:
- Licensed Nurse Practitioner (NP) or Physician Assistant (PA) in Utah.
- Extensive experience in wound care (home health, SNFs and/or clinic setting preferred).
- Proven ability to handle a large patient panel with a high degree of independence.
- Strong networking and relationship-building skills within the Salt Lake City medical community. (SNFs, home health, referring providers).
- Ability to provide exceptional references (we will verify).
