    • Job Summary

    We are a new private-duty home care agency seeking a compassionate and dedicated Registered Nurse (RN) to oversee client care and care professional operations. The RN will ensure high-quality care through client / home assessments, care plan development, and care professional (Certified Home Health Aides) supervision, in compliance with New Jersey regulations.

    Responsibilities

  • Collaborate with clients / designated family members, and colleagues to create and implement patient care plans
  • Assist with hiring qualified and compassionate care professionals
  • Train and oversee all care provided by Dharma Home Care care professionals
  • Make site visits to meet current and prospective clients, conduct home assessments, and ensure care professional compliance with the care plan and best practices
  • Document any changes in patient conditions and adjust care plan accordingly
  • Serve as a liaison between clients, families, and the agency, addressing concerns and ensuring satisfaction
  • Maintain accurate records of patient care and discuss observations with clients / family members
  • Assist with gaining and maintaining Commission on Accreditation for Home Care accreditation
  • Ensure compliance with all state regulations, agency policies, and accreditation standards

    • Qualifications

  • Bachelor’s Degree in nursing, social work, gerontology, psychology, health and human services, or other related field of human services OR 2 years’ experience in geriatrics and / or care management required
  • Active RN license in good standing with the New Jersey Board of Nursing.
  • Previous experience as a Registered Nurse is preferred
  • Excellent interpersonal and client care skills
  • Valid driver’s license
  • Ability to travel within Bergen County to conduct visits (reliable transportation required)
  • First aid / CPR certification required
