CASE MANAGER JOB DESCRIPTION
REPORTS TO: CLINICAL MANAGER
JOB GOAL:
Provides and leads the care of the patient and family to achieve the highest quality clinical outcome and patient and family satisfaction through effective and efficient use of resources.
MINIMUM QUALTIFCATIONS:
- RN or PT with at least 1 year of nursing or therapy experience with 1 year in home health care preferred. Leadership or supervisory experience preferred.
- Demonstrated excellence in clinical assessment, clinical judgment, time management, communication, and collaboration.
- Ability to evaluate the effectiveness of the plan of care.
- Ability to manage collaborative decision-making during the entire length of stay.
- Possesses strong communication and organizational skills.
- Skilled in documentation, patient assessments, time management, critical thinking, and productivity standards.
- Self-directed and innovative, empowered, and accountable.
- Prolonged or considerable walking and standing. Able to life, position, or transfer patients. Able to lift supplies and equipment. Considerable reaching, stooping, bending, kneeling, or crouching. Visual acuity and hearing to perform required nursing skills.
- Must be a licensed driver with an automobile that is insured in accordance with state or organization requirements and is in good working order.
JOB RESPONSIBILITIES:
- Leads the clinical care decisions for assigned group of patients and clinicians.
- Evaluates the effectiveness of the plan of care and implements necessary changes while communicating the plan to other members of the care team (Pod).
- Understanding of Medicare admission criteria and eligibility.
- Assessment of patient/caregiver needs for OASIS and throughout episode (performs all OASIS events).
- As SOC clinician, documents patient/caregiver involvement in the initial plan of care, performs medication reconciliation, completes medication profile and/or reviews existing profile on first visit.
- Ensures that patient home chart contains required documentation at Start of Care and is updated as necessary.
- Develops therapeutic relationship with the patient and patient’s family/caregiver while involving them in the patient’s care.
- Communicates with the physician at start of care and ongoing regarding the patient’s needs and reports any changes in the patient’s condition; obtains and receives physician verbal orders as required.
- Knowledge of state/federal regulations, evidence-based practice standards, and coordination of care and community resources.
- Ensure/audits that the 485, Medication profile and schedule/calendar are in each patient home chart and clinical documentation is evident of review with patient/caregiver.
- Weekly Pod update and Case Conference with Clinical Manager.
- Communication with patient, family care team and community resources.
- Care follow-up and re-assessment throughout the episode.
- Discharge Planning and scheduling in collaboration with Pod members.
- Appropriate frequency utilization based on patient assessment need and best practice.
- Supervision of Home Health Aides.
- Participates in on-call duties and weekend rotation as defined by the on-call policy.
- Maintains an adequate trunk stock of supplies and equipment.

Seva Hospice is a locally owned and operated company of experienced hospice providers with a desire to serve the communities we live and work in. Due to growth, we are looking for a Registered Nurse Case Manager to join our dynamic team to provide care and support to terminally ill patients. This is a full time position, primarily serving Stanislaus County and surrounding communities.

For this role, you need to be a registered nurse who sees the value of hospice care in alleviating pain and suffering. A compassionate nature and excellent interpersonal skills will make you a good candidate for this position. We expect you to be knowledgeable in caring for the terminally ill, providing expert symptom management and providing emotional support. The goal is to help patients live their final days with dignity and comfort.
