We are seeking Case Managers to join the GetWell Health System. GetWell is committed to making a difference in our community and helping to improve the lives of our patients. Our talented and compassionate team strives to assist in high-quality behavioral health care. The Case Manager will evaluate patients in-office consultations and via video teleconference. The Case Manager will be able to create his/her own schedule with the minimum of 20-hr/wk.About the PositionThe Case Manager will provide assessments and support individuals in gaining access to needed medical, social, educational and other needs. Case Management services may include interactions with family members, other individuals and other community organizations. The Case Manager is responsible for providing assistance, directly and indirectly, to obtain access to services and coordination of care.Required TrainingSuccessful completion of Targeted Case Management certification within six (6) months of employment;Successful completion of continuing education.Duties and ResponsibilitiesEvaluate patient needs via video teleconference consultations.Direct and collaborate with other agencies to achieve individualized goals.Gather and maintain patient information and records, including social and medical history obtained via self-report.Prepare and submit assessments, progress notes, reports and summaries through our electronic health records (EHR) system.Perform direct services/consultations via telephone in the event that teleconferencing equipment is temporarily inoperable.Follow company policies and procedures for the patient’s safety.RequirementsA relevant degree from an accredited college or university; or be a CADC or CADAC;Professional liability insurance – with minimum limits of $1,000,000 per occurrence and $3,000,000 aggregate.ExperienceOne Year: 1 year (Preferred)Position TypeJob Types: Full-time, Part-timeAbout GetWellWe are an outpatient office that offers different behavioral health and addiction treatment options.
