Diabetes Reversal Health Coach (Telemedicine)

Position Overview:
We are looking for a trainable Health Coach to join our team and provide personalized, one-on-one support to patients with Type 2 Diabetes through telemedicine. In this role, you will coach approximately 100 patients at any given time, guiding them through their health transformation with regular check-ins, ongoing motivation, and expert advice. You will work with patients primarily via email, phone, and/or video consultations to help them manage their diabetes and embrace healthier lifestyle choices.

Key Responsibilities:

• Provide Telemedicine Coaching: Offer personalized coaching to DRG patients, providing guidance and support via email, phone calls, and/or Zoom/telemedicine video sessions.

• Weekly Check-Ins: Ensure that each patient reports at least once a week to discuss their progress, challenges, and successes in managing their diabetes.

• Develop Customized Health Plans: Work with each patient to create tailored action plans based on their specific health goals, medical history, and lifestyle factors.

• Track Progress & Adjust Plans: Monitor patient progress and adjust health plans accordingly, offering motivation, encouragement, and evidence-based recommendations.

• Patient Education: Educate patients on nutrition, exercise, stress management, sleep hygiene, and other lifestyle factors that play a role in diabetes management and reversal.

• Documentation & Reporting: Keep accurate and up-to-date records of patient interactions, progress, and feedback.

• Patient Support: Provide ongoing support through questions, concerns, and troubleshooting to ensure patient success and satisfaction.

• Collaboration: Work closely with our sales, administrative, and medical teams to ensure continuity of care and consistent patient outcomes.

All aspects of the position will be trained on.

About You:

• Passionate About Health & Wellness: You have a genuine passion for helping individuals manage and reverse Type 2 Diabetes through lifestyle changes.

• Strong Communication Skills: You are an empathetic and motivating communicator, able to offer clear, actionable advice while maintaining a supportive and encouraging tone.

• Self-Motivated & Organized: You can manage a large patient load, stay organized, and remain self-disciplined while working remotely.

• Comfortable with Technology: You are proficient with telemedicine platforms, email communication, and other digital tools.

• Goal-Oriented: You are committed to helping patients achieve real, measurable health outcomes and are motivated by seeing their success.

• Own Workstation/Computer: You have a reliable workstation, computer, and high-speed internet access to effectively manage patient interactions and stay connected with our team.

Compensation:

• Base Salary: $5,000 - $6,000 per month

• Patient Load: Approximately 100 patients at any given time

Work Hours:

• Flexible work schedule: (Monday -Friday) with the expectation of providing consistent support to patients, including at least one check-in per patient each week.

• Remote work – you can manage your own time and workspace.

Requirements:

• Experience working with patients 50 and a strong understanding of age-related health concerns is a plus.

• Excellent interpersonal skills with the ability to motivate and build rapport with patients virtually.
