What are the responsibilities and job description for the Case Manager II (Family Based Counseling) position at Diakon?
General Responsibilities:
- Responsible for serving as a resource person, case manager, counselor and trainer to support the Family Based Services Unit.
- Responsible for providing in-home individual and family therapy to clients with social and emotional disturbances.
- Creates systematic change through the use of a variety of therapeutic interventions with clients and their families.
- Provides case management and crisis intervention as needed.
- Childline & DHS FBI clearance results are required prior to beginning employment.
Education/Training: A Bachelors degree in social service or a related field of study required.
